Many thanks to those who have tried to take this survey. I have reported the problem and we are waiting to hear how it may be resolved. I will let you know when I hear something further...

The survey is explained in my previous Ataxia UK blog. This reply is in response to the comments on that blog post. I posted this update in a separate blog to ensure as far as possible that all the people that commented on the previous blog saw it.
